Patient Medical History Questionnaire (Word): For new patients or returning patients that we have not seen for more than two years. We prefer that you complete the online medical history questionnaire. A link should be sent to your email address. If for some reason you do not receive the link or are unable to complete the online questionnaire, you may print this form, complete it and bring it with you to your appointment.
